2. How it came to light
We received reports from our business partners that similar defects were occurring at multiple retailers. Upon opening and inspecting our inventory internally, we discovered that some ball chains were the wrong size for the two types of jump rings mentioned above.

The cause was that ball chains intended for a different product got mixed into the production line for the product in question at a manufacturing plant in China, and were then packaged as part of the product.
